New NICE Guideline published on Hospital Transfers

NICE has recently published a guideline on the transfer of individuals between hospitals and home. This forms essential reading for healthcare workers and it should be noted that CQC uses NICE guidelines to inform the inspection process.

The guideline makes several recommendations regarding care plan development prior, during and after admission. It states that healthcare commissioners and carers should regularly review care plans and discharge should be in the forefront of their minds when such reviews are carried out. The pervasive principles of good person-centred care run throughout the guideline.

It is hoped that care providers are mindful of this guideline when planning care provision and advance planning is undertaken to ensure smooth transitions between hospital and home settings. The guideline provides a basis from which a healthcare organisation can tailor its policies regarding Hospital transfers.
